- Formerly known as the National Framework for Documenting Care in Residential Aged Care Services, this is a collection of tools and resources for communicating and delivering high quality care. It contains tools for initial assessment of a resident and tools for the assessment of behaviour, cognition, continence, falls, physical mobility and ADL, self administration of medicines (this is the APAC tool), nutrition and oral and dental health, pain, skin, wound and social, cultural and spiritual domains. This also contains a list of tools included in the package and a care profile document.
