The Merck Manual of Geriatrics
A regularly updated resource that includes suggestions for monitoring, assessing and treatment of disorders and diseases in geriatric patients. The link takes you directly to the contents page. The resource is organised by system, with some introductory chapters covering basics of geriatric care, falls and fractures and rehabilitation.
RACGP Silver Book – The medical care of older persons in residential aged care facilities
Available as a pdf resources from this site, the RACGP Silver Book contains an excellent section on the clinical care of older persons. The book also provides access to a number of clinical tools (for example, the Abbey Pain scale) and some non clinical tools to facilitate working relationships between RACF and GP providers.

Formerly known as the National Framework for Documenting Care in Residential Aged Care Services, this is a collection of tools and resources for communicating and delivering high quality care. It contains tools for initial assessment of a resident and tools for the assessment of behaviour, cognition, continence, falls, physical mobility and ADL, self administration of medicines (this is the APAC tool), nutrition and oral and dental health, pain, skin, wound and social, cultural and spiritual domains. This also contains a list of tools included in the package and a care profile document.
