Monitor / Evaluation

Monitoring and evaluation of medication management incorporates:

• Monitoring, evaluating and reporting therapeutic effects of medicines through processes.
• Feedback and comment to and from residents, family and other health professionals.
• Monitoring for, recognition of and appropriate response to adverse effects.
• Monitoring, recording and reporting of adverse drug events.

 

Knowledge

Demonstrate understanding of:

• pharmacology (reflected in level of education)
• application of knowledge, and
• decision making.

Skills

Demonstrate ability to:

• undertake a clinical exam and record findings
• monitor and appropriately manage (including reporting) the outcomes (including side effects) of medicines, and
• investigate, manage and report medication incidents.

Tasks

• Undertake clinical examination and accurately record findings.
• Apply knowledge of pharmacology when making and recording clinical decisions.
• Report adverse drug events.
